Description

This is a legal form  that you can modify to help cover yourself when you are teaching Qigong.  Some places that you rent will request this, the business that you work at, or your insurance company.  This form is editable and I would recommend that an attorney determine if any additions are needed for the law in your area.  Hand these out to your students to cover yourself in case of a lawsuit.
